Special mention to the optician there, she was very thorough and patient while checking and gave extra time to do all the required tests to eliminate anything untoward. The GP service didn't have one either, even though they should. So I took a punt at getting one from Pyramid Pharmacy in Bermondsey. Not only did they have the form, but they provided it with a smile, unlike my local pharmacy and more worryingly the grumpy receptionists at my GP surgery. So, top marks, you wonderful women.
