Community Case Management Services Ltd

CCMS Ltd provides rehabilitation services and quality of life solutions for adults and children who have suffered catastrophic/complex and life-changing injuries. We work with experienced Case Managers who are registered professionals with the NMC or HCPC. The Case Manager assesses and implements individually designed plans, focusing on enhanced rehabilitation and independence for our clients. We provide: Initial Assessments Reports, Case Management and Employment Services for clients directly employing their own care and support personnel. We work to all regulatory requirements and are CQC registered.
